There’s an old saying: “Give a man a fish and you feed him for a day. Teach a man to fish and you feed him for a lifetime.”

Several STS members are taking that maxim to heart by participating in charitable mission trips to developing countries. Not only are they performing dozens of life-saving surgeries, but they are also educating the local medical staff

“The most striking thing is that we encounter a lot of problems that we very rarely see in the United States,” Dr. Jacobs said. “In the United States, babies with tetralogy of Fallot usually have heart surgery by the age of 6 months. In Jamaica, it’s not unusual to repair children with tetralogy who are 5 or 6 years old, and occasionally even 11, 12, or 13 years old. Usually, prior to surgery, these older children with tetralogy are quite ill, but after surgery, they become very healthy!”

Although the role of an anesthesiologist in Jamaica is not dramatically different from that in the United States, Dr. Vener said he did have to make a few adjustments.
“You have to scramble and use a MacGyver technique sometimes, trying to find one type of monitor from one place and a different type of monitor from another place and putting it all together to get a working set of consistent monitors,” he said.

“In the pediatric age group, there’s a lot of pulmonary infectionand destroyed lungs because they have poor treatment for pulmonary infections. Tuberculosis is really common. They have a lot of children with empyemas requiring decortication,” he said.

Dr. Crosby quickly learned the importance of adapting to the circumstances, getting back to basics in many regards. “Typically in lung surgery, you use staplers to staple along the edges so it doesn’t leak,” he said. “They don’t have that there, so you have to manually sew things.” Since the hospital’s portable x-ray machine was broken
for most of his stay, they had to disconnect patients from ventilators and hand-bag them for trips to the X-ray Department. Despite the lack of technological advances, Dr. Crosby said the enthusiasm of the Rwandan surgical team was outstanding.

“This is one of the most rewarding things a thoracic surgeon can do. While not easy, it is very rewarding to know that we have the privilege of affording these children and their parents a very different outlook on life,”

DENVER – After a failed first ablation procedure for paroxysmal atrial fibrillation, redo ablation proved more effective than did antiarrhythmic drug therapy in a randomized trial, Dr. Jonathan Steinberg reported at the annual meeting of the Heart Rhythm Society.

The success rate of a first ablation procedure in patients with symptomatic paroxysmal AF is typically about 60%. The question of what to do for the 40% who are nonresponders has been unclear, with no prior randomized clinical trial evidence available to guide decisions, noted Dr. Steinberg of Columbia University, New York.

Als iets snel moet je bij chirurgen zijn. Als je bijvoorbeeld een anesthesist achter de aardappels hebt zitten bij moeders de vrouw doen ze daar al gemiddeld 2 uur over.

40 jaar doen over eerherstel van Bob valt op zich dan wel mee.

Ik neem aan dat hij erelid is geworden. Als ze hem een factuur sturen voor de contributie is het niet onmogelijk dat de NVA de deurwaarder naar Veldhoven moet sturen.

Toen Smalhout zijn inaugurele rede hield was ik nog maar net begonnen met specialiseren. Ik mocht toen af en toe meekijken hoe de grote chirurgen het deden. Toen was het heel normaal dat de chirurg een uur lang mij verrot aan het schelden was omdat de wereld naar de klote ging omdat een vrouw dacht dat ze wel chirurg kon worden. Immers vrouwen moeten maar zuster worden. Toen ze wisten wat Smalhout had gezegd was ik ineens van alle vernederingen af, Smalhout was ....vul zelf maar in.

Cardiovasculair risico kun je net iets nauwkeuriger voorspellen door niet alleen de bekende cardiovasculaire risicofactoren te meten maar ook met CT de coronaire calciumscore te berekenen (A6123). Dat is belangrijk, maar wat zijn de consequenties van deze bevinding (A6459)? Gaan mensen eindelijk afvallen en meer bewegen omdat er kalk te zien is in hun coronairen? Voordat er een indicatie is voor zo’n dure test moeten er nog veel vragen worden beantwoord. Velen zullen daar niet op wachten. En al is de test kostbaar, met de nieuwste apparatuur is hij wel in een wip gemaakt. Uitzoeken of mensen nu echt beter af zijn met die kennis over hun coronaire kalk is heel wat complexer. Daarvoor zijn grote vergelijkende studies nodig met jaren follow-up waarin bijgehouden moet worden welke gebeurtenissen optreden. Wie moet die studies betalen? Behalve de patiënt en de belastingbetaler heeft niemand hier belang bij. En mocht het toch een keer lukken dan is er tegen de tijd dat de onderzoeksresultaten beschikbaar zijn allang weer een nieuwe andere beeldvormende of veelbelovende ‘point-of-care’-test.

Mooie glanzende apparaten hebben enorme overtuigingskracht.
