Human infection with influenza A(H7N9) virus in China - update

As of 13 April 2013 (17:30 CET), the National Health and Family Planning Commission notified WHO of an additional six laboratory-confirmed cases of human infection with influenza A(H7N9) virus. Of the latest laboratory-confirmed cases, one is from Beijing, one from Shanghai, two from Jiangsu and two from Zhejiang.

The first patient reported from Beijing is a seven-year-old girl who became ill on 11 April 2013. The patient from Shanghai is a 56-year-old man who became ill on 1 April 2013. The two patients from Jiangsu are a 77-year-old woman who became ill on 5 April 2013, and a 72-year-old man who became ill on 1 April 2013. The two patients from Zhejiang are a 65-year-old man who became ill on 3 April 2013 and a 38-year-old man who became ill on 6 April 2013.

To date, a total of 49 patients have been laboratory-confirmed with influenza A(H7N9) virus in China; including 11 deaths. More than a thousand close contacts of the confirmed cases are being closely monitored.

Investigations into the possible sources of infection and reservoirs of the virus are ongoing. Until the source of infection has been identified, it is expected that there will be further cases of human infection with the virus in China. So far, there is no evidence of ongoing human-to-human transmission.

WHO does not advise special screening at points of entry with regard to this event, nor does it recommend that any travel or trade restrictions be applied.

This formal notification and publication follows verification of the information, and may therefore come after, or not include, some cases reported through public media and other sources.

2. To date, there is limited information to determine whether the reported number of cases represents some or all of the cases actually occurring. As some relatively mild cases of illness have now been reported, it is possible that there are other such cases that have not been identified and reported.

3. If the current pattern of sporadic infections continues, WHO will cease frequent reporting of case numbers, and focus its Disease Outbreak News on new developments or changes in the pattern or presentation of infections.

A recent outbreak of a previously unrecognized novel reassortant avian influenza A (H7N9) virus in China had resulted in 131 documented cases and 36 deaths as of May 16. Although some patients had a history of contact with live poultry or visiting live-animal markets before the onset of illness,3 the source of infection remains unclear.

In Tzum (Friesland) is bij een pluimveebedrijf met zo'n 9000 legkippen vogelgriep (aviaire influenza, AI) vastgesteld. Dit bedrijf heeft 1 stal met buitenuitloop. Het gaat waarschijnlijk om de milde H7-variant.

Omdat een laagpathogene (milde) H7 variant kan muteren tot een hoogpathogene (zeer besmettelijke en voor kippen dodelijke) variant, moet het bedrijf zowel bij een laag- als een hoogpathogene variant worden geruimd. Dit gebeurt op basis van Europese regels. De ruiming wordt uitgevoerd door de Nederlandse Voedsel- en Warenautoriteit (NVWA).

Maatregelen

Vanaf zaterdag 3 augustus 9.00 uur geldt er in een zone van ruim 1 kilometer rond het bedrijf in Tzum een vervoersverbod voor pluimvee, eieren, pluimveemest en gebruikt strooisel. Binnen het gebied liggen geen andere pluimveebedrijven.

Vogelgriepvirus kan van mens op mens
Het vogelgriepvirus H7N9 is voor het eerst overgedragen van mens op mens. Dat melden Chinese virologen en epidemiologen (Xian Qi e.a.) in BMJ van deze week.
Ze beschrijven de casus van een 32-jarige vrouw die overleed na besmet te zijn geraakt door haar 60-jarige zieke vader.

New bird flu 'has unique traits'
The new flu which has emerged in China has unique traits, say scientists.
It is able to infect both the nose, giving it the potential to spread easily, and penetrate deep in the lungs where it causes pneumonia.
The authors of the American Journal of Pathology say the twin attack has not been detected in previous bird flus.
Meanwhile, a separate study has taken the early steps towards a vaccine for another emerging virus, Mers-coronavirus.
There have been 135 people infected with avian influenza A(H7N9) and 44 deaths since the outbreak started in Spring.
However, restrictions on live poultry markets have largely curbed the number of infections.
The study, by the Erasmus University Medical Centre in The Netherlands, looked at what parts of the body the virus could bind to and infect.
Infections like the common cold spread easily as they infect the upper respiratory tract, the nose and throat, so sneezing releases a lot of viruses into the air.
Other more deadly infections, such as the H5N1 bird flu, infect the lower respiratory tract deep in the lung where they can cause deadly pneumonia.
One of the researchers Prof Thijs Kuiken told the BBC the new bird flu could do both: "This has not been shown for this virus before.
"The study points to the fact that the virus has the potential to transmit easily in people and give pneumonia."

In Scheemda is vogelgriep vastgesteld op een biologisch pluimveebedrijf met rond 12.000 legkippen. Dit meldt het ministerie van Economische Zaken. De dieren worden binnenkort geruimd.
